How did this patient hear about The Paramount at Somers? Hospital Referral

Details of Experience:
Hector Vazquez is a 51-year-old who admitted to The Paramount at Somers on 4/17/20 following an acute care hospital stay on 4/12/2020 for respiratory failure, pneumonia secondary to COVID-19.

Hector had a previous medical history of asthma and multiple sclerosis. He lives at home with his fiancé on the first floor of a three-family home. Hector was previously independent with all ADLs (activities of daily living), and did not require the use of supplemental oxygen; however, when he arrived at The Paramount he was on 3 liters of oxygen in order to maintain an oxygen saturation above 90%.

Upon initial evaluation, Mr. Vazquez presented with a decline in endurance, activity tolerance, functional mobility, ability to complete ADLs, and balance. Additionally, he presented with a productive cough and shortness of breath during light activity. Due to his physical impairments and functional deficits, Hector was at high risk for falls, a decline in the ability to participate in functional tasks, and a decreased level in overall mobility. Hector’s goal for therapy was to increase independence and return home without supplemental oxygen.

Hector remained a highly motivated patient throughout his stay at The Paramount. He was always ready to participate in his therapy sessions and maintained a positive outlook. He began his therapy journey with the use of a front wheeled walker, due to his decreased balance and activity tolerance. His oxygen saturation would drop to 83% on room air with light activity. He required frequent rest breaks secondary to fatigue and shortness of breath. Hector was trained in proper breathing techniques and the use of a spirometer throughout the day. Through daily gait training sessions, as well as specific therapeutic exercises and activities, he made consistent functional gains, built total-body strength, increased his endurance, and gradually decreased his need for supplemental oxygen over the course of his stay, as he was able to consistently keep his oxygen saturation over 90% during his exercise sessions.

Through hard work, staying focused, remaining highly motivated, and always maintaining a positive attitude, Mr. Vazquez made immense progress throughout his therapy journey. With the help and care from the staff of The Paramount at Somers, Hector was successfully able to discharge home to his loving fiancé, family, and friends without the use of any assistive devices or the need for supplemental oxygen. Hector made a full recovery and walked out of The Paramount to a cheering crowd of family, friends, and staff. The team at The Paramount at Somers is proud to have been a part of his recovery journey.
