Artina W. is a 59-year-old female with developmental delays who resides in a group home. She was admitted to the Paramount at Somers Rehabilitation & Nursing Center following a right total knee replacement due to severe arthritis. Upon admission on April 15, Artina required extensive assistance with nearly all aspects of mobility and self-care. Her rehabilitation journey demonstrates the comprehensive, multidisciplinary approach that the Paramount provides to every patient, regardless of complexity.
At the time of her physical therapy evaluation, Artina was dependent for bed mobility and transfers, requiring a Hoyer lift for all transfers. She was unable to ambulate and could not safely navigate stairs. Over the course of her being here, Artina made tremendous progress through consistent physical therapy, nursing support, and interdisciplinary care. She progressed from total dependence to requiring only setup assistance for bed mobility and transfers. She is now independently ambulating over 200 feet with a rolling walker and has been cleared to walk throughout the facility at her discretion. In addition, she can now confidently navigate a full flight of 7–10 stairs with supervision.
Artina also achieved significant gains through occupational therapy. Upon admission, she required moderate assistance with personal hygiene and oral care, was dependent for bathing, required moderate assistance with upper-body dressing, was dependent for lower-body dressing, and was dependent for toileting transfers and hygiene. Through individualized treatment and repeated practice of activities of daily living, she steadily increased her independence. Currently, Artina independently feeds herself, completes personal hygiene with only verbal cueing, requires supervision for upper-body dressing, and moderate assistance for lower-body dressing. She now performs toileting transfers with setup assistance. While she still requires assistance with thorough toileting hygiene, this level of support is consistent with the services available at her group home.
Although Artina’s verbal communication is limited, she quickly became a favorite among staff members throughout the facility. Her warm smile and positive demeanor brightened the days of those around her, and she consistently demonstrated a willingness and eagerness to participate in her therapy sessions and daily care. What makes Artina’s success especially meaningful is that her progress extended beyond physical gains alone. Her confidence, participation, and engagement grew throughout her stay. As Artina was discharged on June 22, her progress highlighted the power of a coordinated rehabilitation program. What began as a patient who was completely dependent for mobility and many daily activities resulted in a resident who could independently ambulate throughout the facility, safely manage stairs with supervision, and perform most personal care activities with minimal assistance. Her outcome reflected not only the success of her knee replacement rehabilitation but also Paramount’s commitment to helping every individual reach their highest level of function and independence. While we will certainly miss seeing her smile around the facility, we are proud of everything she accomplished and wish her continued success as she returns home.
